Personalised Care and Support Plan

In February 2023, during a focus group, we found that the nationally required Personalised Care and Support Plan was not being embraced by local maternity staff, and service users often hadn’t seen it or didn’t understand it.

We brought together service users and community teams to reimagine and coproduce this important planning document.

We used the core values and principles of the Kernow Parenting Journey to make sure the document uses accessible language, imagery where appropriate, time appropriate information, and consistent and clear messaging throughout the parenting journey to support informed decision-making.

The Personalised Care Plan work will inform development of the digital offer for parent ‘Natters Bitesize’ video resources and the parents website development, which will be branched out in specialist documents for neonatal care and the Isles of Scilly.
